This is simple to put together! Just print out the image at the bottom of this blog post. Attach the little pieces to Hersey's Nuggets. Put them in a bag and top it off with the bag header.

The image at the bottom is the printable. You click on the image to bring it up larger in your browser. Then you can drag the image right onto your desktop or control+click / right click and select "save image as".
